. on september 11th the u.s. ambassador and three other americans were killed on a terrorist attack in libya. tuesday an independent review investigating the incident released its findings. a panel was highly critical of the state department saying they ignored arrests in tripoli for more guards and security upgrades relied who heavily on specific requests instead of security. and used unprotect militia to protect the compound. the report also found the attack was not the result of protests over anti-islamic video as first reported. in a letter to congress, secretary of state hillary clinton said she accepts all of the panel's 29 recommendations writing there is no higher priority for me or my department. >>> though there seems to be progress still no fiscal cliff deal in washington. house speaker john boehner said he has a backup plan. but that's not enough for one in three americans who according to a new survey are cutting back on spending due to fears of going over that cliff. nbc's tracie potts joins us now with more. good morning. >> reporter: mara, good morning. good morning, everyone

to have you with us. it is 5:00 a.m. in the east. up first, systemic failures at the state department. that is a scathing conclusion of an independent review board investigating the deadly september 11th attack at the u.s. consulate in benghazi. ambassador chris stevens and three other americans were killed that day. the panel finding security in benghazi was grossly inadequate to cope with the attack, that washington ignored repeated requests to beef up personnel, and ultimately a lack of transparency, responsiveness and leadership senior levels both this washington and in libya. alise, what do you make of this report. a lot of these findings we've been talking about for a very long time. >> that's right, but also in the last few months, there's been so much politicization of this attack, especially with these talking points and the whole issue of susan rice, ambassador susan rice and what she said about whether there was a protest. this report didn't find that there was a protest, but it also kind of really is the first look at what went wrong, a kind of very sobering look i'd say o

enough time discussing ways to help them assimilate into civilian life. as the son of a u.s. air force veteran who spent 31 years in the air force, i'm acutely aware, as coul kay is, that it t just those that wear the uniform that serve, but their families as well. many returning vets and their families encounter a whole range of social and economic hardships that can be hard to overcome. most notably, the unemployment rate among our returning vets from afghanistan and iraq is significantly higher than for the general population, something i know kay has worked on extensively. she's also worked to get our veterans the medical assistance, the job training and the financial support they need. indeed, i don't know of any senator that's done more to help america's heroes adjust to life after the military. that's just one of the reasons why she will be sorely missed. here's another reason, though: kay has fought time and time again to promote tax relief for hardworking texas families. in thehooin the mid-1990's, shed create the so-called homemaker ira to make sure that stay-at-home moms and

but did not single out any individual u.s. individuals for blame. the review board was headed up by retired ambassador thomas pickering and former chairman of the joints chief of staff, admiral mike mullen. they'll discuss their findings with lawmakers today. the panel blamed systematic failures and leadership and management deficiency in two bureaus of the state department for security measures at the benghazi compound that were described as inadequate. the report concluded that while there were no specific threats of a terrorist plot, there were several incidents leading up to the september 11th attack that should have served as warning signs. investigators also praised the diplomatic security agents who responded to the attack and absolved the pentagon for any blame, saying there was not enough time for military assets to have made a difference. the board also made 29 recommendations to improve security at u.s. diplomatic missions around the world. they include increasing the number of marine guards at each post, relying less on local security forces, and hiring more highly tr
